CHECKLIST FOR CHOOSING AN ADULT DAY CARE FACILITY:

§         Location of facility

    • Is the facility accessible?
    • Garden Oaks is accessible for ambulatory and wheelchair bound individuals
    • Is transportation to the facility available?
    • Facility transportation is available for individuals who live within a 15 mile radius of the Center
    • Is there an extra charge for transportation?
    • There is no extra charge for facility transportation

§         How long has the day care facility been in business?

§         Garden Oaks Adult Activity Center has been in the present location for 14 years

§         What days and hours is the facility open?

§         The Center is open Monday through Friday from 7:00am until 5:00pm

  • Is the day care licensed by the state health department or department of social services?
  • Garden Oaks is licensed by the State as a DAHS (Day Activity and Health Services) facility

§         Has the state health department received any substantiated complaints about the care provided in the facility?

§         Garden Oaks has no complaints with the State or any other organization

§         What is the cost of care? Hourly, daily, or weekly? Must you commit to a minimum amount of service, i.e., at least 2 days/week? Are all activities included in the cost?

§         The cost of care is dependant on the Level of Need.  We accept Medicaid, and Private Pay.

§         Billing is done on a monthly basis.  There is no minimum commitment of day per week and all activities are included in the cost.

§         Does the facility accept individuals who:

    • Are incontinent?            Yes
    • Are in wheelchairs?       Yes
    • Have memory loss?       Yes, depending on degree
    • Have difficulties in speaking? Yes
    • Wander?            Yes, depending on degree
    • Have special dietary requirements?     Yes
    • Have behavioral problems?        Yes, depending on degree

§         What is the participant to staff ratio? Does this ratio meet state standards? What professional staff is available to help with care planning and concerns?

§         The participant to staff ratio is 1:8 which meets state standards and we have a full time LVN on staff as well as an RN consultant for 4 hours weekly.

§         What activities are provided?

    • Are there individual and group activities? Yes, Please see attached sheet
    • Are the activities stimulating? Exercise, music, crafts, memory sharing, etc. Yes, Please see attached sheet
    • Are individuals with dementia separated from other participants or included in activities?
    • They are included in activities and they also have some separate activities

§         Are the meals well-balanced and enjoyed by participants? Are meals included in the cost?

§         Meals are balanced and monitored by a Licensed Dietician and regulated by the CACFP (Child and Adult Food Program), and they are included in the cost

§         Is staff willing to spend time finding out what you want and need? Were the programs and activities explained?  

§         A complete interview is done with the family members and client inclusion is encouraged

§         Is the day care facility clean, odorless, and pleasant to visit? Does the facility have comfortable furniture for activities and for relaxation?

§         We work very hard to assure a clean and pleasant and comfortable surrounding for the clients

§         Do participants interact with each other? Do they seem to enjoy their time at the facility?

§         We encourage socialization and have many activities for everyone

§         Is the staff pleasant to the participants?

§         We employ a very caring staff

§         Can the facility accommodate the following special physical or medical requirements:

    • Dispense medicine?  Yes, There is a full time LVN on duty
    • Give reminders about taking pills? Yes
    • Assist with toileting or handle incontinence? Yes
    • Provide total access and participation to wheelchair-restricted clients?  Yes
    • Effectively communicate with hearing impaired participants?  Yes

§         How do they insure safety? How are behavior problems handled? Are there specific behaviors or care needs which would require your loved one's withdrawal from the program?

§         The facility is secure with door chimes and observation of all doors and outside areas.  Outside areas are all fenced and gated.  Behavior that would endanger the Client or others is grounds for withdrawal from the program.
